When a very similar situation arises, Dr Tenma does not obey orders and continues to operate on a boy, Johan Liebert, with a gunshot wound to the head, instead of on the Mayor, who has arrived at the hospital later. The boy is saved, but the Mayor dies. Because of this, Dr Tenma loses his prestige and position. Strangely, the hospital director and all the doctors in the way of Tenma regaining his position are murdered. Of course, Dr Tenma is the number one suspect. But the authorities have no proof of his involvement in the murders.
Nine years have passed, and Dr Kenzo Tenma is chief of surgery at Eisler Memorial. He performs an operation and saves the life of a criminal, Adolf Junkers. In his postoperative delirium, Junkers talks about a Monster.
When Tenma returns to his patient, he finds the police officer on duty dead and the patient gone.
At this point, a series of events unexpectedly brings Dr Tenma to face the ghosts of his past.
